Submission of information authorizes C&A Media Group, LLC to use your likeness and media in
the interest of booking your act.
Artist agrees that performance opportunities solicited by C&A will be entitled to standard
commission rates of 10% of Gross earnings from said performances.
Email notification of said opportunities will be sent to artist at time of solicitation. Such
performance opportunities solicited by C&A will be considered brokered by C&A and C&A will be
entitled to commission. All commission will be payed to C&A Media Group within 5 business
days of performance by means of check, money order or cash. All checks returned 'non sufficient
funds' will be subject to a $25 NSF fee. Failure to make good on payment within 30 days of
notification of NSF will result in $100 NSF processing fee. Artist agrees in the event legal action
is taken in recovering a NSF check, artist will be responsible for all NSF fees, original amount
and legal fees incurred by C&A Media Group, LLC.
C&A Media Group,LLC holds no legal or financial responsibility for artist, equipment/property or
artists actions to include lewd or criminal acts, property damage and/or financial damages to
venue as a result of artist actions. C&A Media Group,LLC acts as an agent only and not a legal
representative of the Artist.
All performances booked through C&A Booking will be bound by a performance agreement on a
case by case basis and confirmed by email with artist. Cancellation of performance within 4
weeks of performance by artist will result in no penalty. Cancellation of performance within 2
weeks of performance by artist will result in a $15 booking fee due to C&A Media Group,LLC
within 5 business days of cancellation. They following are excusable cancellations within 2
weeks of performance and no booking fee will be incurred: Medical (with doctor excuse), legal
responsibility (jury duty, court summons, etc.), family death (artist/spouce parent, grandparent,
sibling, aunt, uncle), force of nature/act of god (traveling in inclement weather, vehicular
mishap, natural disaster)
